CopilotIQ and Biofourmis Merge to Create AI-Driven In-Home Care Platform

CopilotIQ and Biofourmis announced on Saturday a strategic merger to form an AI-driven platform that delivers in-home care across the full spectrum from pre-surgical optimization to acute, post-acute, and chronic care.

The combined company seeks to provide a single-vendor solution offering comprehensive services throughout the care continuum.

This approach aims to simplify technology integration, reduce security audit complexities, streamline procurement processes, and enhance patient experiences.

The merger addresses challenges faced by hospitals and payors struggling with the complexity and costs associated with managing multiple point solutions for home-based care.

David Koretz, appointed CEO of the newly formed entity, stated that the merger represents a significant step toward transforming healthcare from reactive treatment to proactive prevention, focusing on personalized, data-driven care delivered in patients’ homes.

The integration is expected to drive improved health outcomes at lower costs to the healthcare system.

By uniting their technologies, the companies plan to expand Biofourmis’ capabilities in delivering complex in-home care using FDA-cleared algorithms and technology.

CopilotIQ’s software suite, designed to enhance patient engagement, clinician efficiency, and clinical personalization, will be integrated across Biofourmis’ acute and post-acute offerings.

The combined platform aims to provide coordinated, convenient, and cost-effective care for patients managing chronic conditions.

Leading investors from both companies, including General Atlantic, Openspace Ventures, and Bessemer Venture Partners, are investing in the merged business.

Based in Nashville, Tennessee, CopilotIQ develops a High Frequency Connected Care platform aimed at improving outcomes for older Americans with chronic conditions.

The company combines continuous biomarker data, behavioral analytics, and licensed nursing visits to drive data-driven improvements in patient health. CopilotIQ was named to Fast Company’s list of the World’s Most Innovative Companies of 2024.

Biofourmis is a global technology-enabled care delivery company that enables hospitals, health systems, and payors to extend care to patients at home across all levels of acuity.

Its data-driven solutions leverage clinical devices, artificial intelligence, and a network of virtual clinical teams and in-home services to drive quality outcomes, improve operational efficiency, and reduce costs.
